   The undersigned is submitting an Environmental Notification1Form (“ENF”) to the Secretary of Energy & Environmental Affairs on or before 9/30/2025

   The Secretary of Energy & Environmental Affairs will publish notice of the ENF in the Environmental Monitor, receive public comments on the project, and then decide if an Environmental Impact Report is required. A site visit and/or rem0te consultation session on the project may also be scheduled. All persons wishing to comment on the project, or to be notified of a site visit and/or remote consultation session, should email MEPA@mass.gov or the MEPA analyst listed in the Environmental Monitor. Requests for language translation or other accommodations should be directed to the same email address.    In accordance with the provisions of the Code of the Town of Groton, Chapter 198, the Earth Removal Stormwater Advisory Committee will hold a public hearing on Tuesday, October 7 at 7:00 PM, in the Town Hall, to consider the application submitted by Transom GP Capital Holdings for approval of a Full Stormwater Management for construction of 200 rental units as shown on the plan entitled, “Fox Howe Groton,” dated August 20, 2025, prepared by Dillis and Roy. The proposed project is located at 500 Main Street, Assessors Map 216, Parcels 95 & 96, owned by 500 MG, LLC.

   Copies of the application and plan are on file in the Land Use Department in the Town Hall.    UMass Memorial Health Care, Inc. (“Applicant”) located at One Biotech Park, 365 Plantation Street, Worcester, Massachusetts 01605, intends to file a Notice of Determination of Need with the Massachusetts Department of Public Health for the acquisition of DoN-required Equipment of one (1) computed tomography unit for use at its proposed Satellite Emergency Facility, which will be a satellite of UMass Memorial Medical Center and will be located at 490 Main Street, Groton, 01450 (the “Project”). The total value of the Project based on the maximum capital expenditure is $2,195,943.00. The Applicant does not anticipate any material price or service impacts on the Applicant’s existing patient panel as a result of the Project. Any ten Taxpayers of Massachusetts may register in connection with the intended Application by no later than November 1, 2025, or 30 days from the filing date, whichever is later by contacting the Department of Health, Determination of Need Program, 67 Forest Street, Marlborough, MA 01752.
